The Port Towns Community Health Initiative

Through a contract with Kaiser Permanente Foundation, CommonHealth ACTION provides technical assistance to the Port Towns Community Health Initiative (CHI) in Maryland. The initiative is a partnership between the Port Towns community, Kaiser Permanente, Consumer Health Foundation, and other institutions, to change community conditions that increase the risk of obesity, chronic disease and health inequities in the Port Towns. The CHI theme is Healthy Eating, Active Living, and its goal is to measurably improve health outcomes and/or healthy behavior in a 7-10 year period, and serve as a model community in terms of physical, social, environmental, and economic health. The framework of the inititiative seeks to address nutrition and physical activity, but also acknowledges the critical role of poverty, education, racism, and other social determinants that are the root causes of poor health.
