The event convened newly funded grantees from the Boston Disparities Project, and two MA REACH Centers of Excellence (CEEDs: Boston Public Health Commission and Greater Lawrence Family Health Center) at the Greater Lawrence Family Health Center. The goal of the Summit was to provide an introduction to REACH/DP and provide a framework for understanding and addressing the social determinants of health (SDOH). Ms. Natalie Burke, CHA Principal, conducted a one-hour workshop to educate the participants on the concept of the Social Determinant s of Health. Participants attended in “Leadership Teams,” 2-4 people from the grantee organization and their partner organizations who would lead the grant work in their communities. Each team engaged in dialogue with one another about their current work, their proposed work with this funding, and how they see the function and utilization of the interstate learning collaborative coordinated by the BPHC and GLFHC.
