Healthy Montgomery Community Health Improvement Process
The Institute for Public Health Innovation is supporting Montgomery County, Maryland with the design and implementation of their health needs assessment and prioritization process called Healthy Montgomery. The purpose of Healthy Montgomery is to identify and characterize disparities in health status among the County's communities and populations, identify unmet needs, plan and implement strategies to meet those needs, and evaluate the effectiveness of the strategies. As part of its role, IPHi designed and implemented a series of nearly 20 community engagement sessions, called Community Conversations, in order to gather the perspectives of County residents regarding health and quality of life. In the fall of 2011 and into 2012, IPHi will facilitate a process of issue prioritization and action agenda development. Healthy Montgomery is funded by the Montgomery County Department of Health and Human Services and the five major hospitals in the county.
